Employee Performance Review Template
A free, editable Word document built for managers running regular one-on-one performance conversations — not a generic all-purpose template.
A performance review that’s just a vague "doing great, keep it up" helps no one. This template forces specificity — real achievements, one concrete area for growth, and goals for next period.
It includes an optional simple rating scale and space for the employee’s own comments, so the review is a conversation, not just a one-way evaluation.
This template is provided for general informational purposes and does not constitute legal, financial, or HR advice. Have it reviewed by a qualified professional before using it with a real client or employee.
Free, editable, ready to fill in. Enter your email and it'll start downloading right away.
Once this is in place, startbuddi for startups handles the client, invoicing, and follow-ups that come after — automatically.

How often should performance reviews happen?
Quarterly is common for fast-moving teams; twice a year works for more established roles. What matters more than frequency is that feedback also happens informally in between, not just at review time.
Should ratings be included?
They're optional here for a reason — some managers find a 1-5 scale useful for tracking trends over time, others find it reduces a nuanced conversation to a single number. Use your judgment.
